QIDS-SR Depressed Test


QIDS-SR – Quick Inventory of Depressive Symptomatology – Self-Report – The QIDS-SR was created by researchers at the University of Texas Southwestern Medical Center. This depressed test looks for symptoms of major depression by using and measuring 16 different factors across 9 different criterion domains.

Please answer each question honestly on how you felt and behaved in the last week. (Please see our Disclaimer).

  1. 1.) Falling Asleep:

    2. I never take longer than 30 minutes to fall asleep

    I take at least 30 minutes to fall asleep, less than half the time

    I take at least 30 minutes to fall asleep, more than half the time

    I take at least 60 minutes to fall asleep, more than half the time

     

  2. 2.) Sleep During the Night:

  10. 10.) Concentration/Decision Making:

    Most of the time, I struggle to focus my attention or to make decisions

    I cannot concentrate well enough to read or cannot make even minor decisions

Each of the four possible answers to each quiz is given an ascending numerical value from 0 to 3, and the total test score is the sum of the following:

Screening test scoring ranges:

0-5 = No Depression Likely
6-10 = Possibly Mildly Depressed
11-15 = Moderate Depression
16-20 = Severe Depression
21 or Over = Very Severe Depression
